Microvascular decompression of the trigeminal nerve requires sharp dissection of all arachnoid around the trigeminal nerve and superior cerebellar artery. The most common vessel found is a rostroventral superior cerebellar artery loop, which compresses the trigeminal nerve either at the brainstem or distally.
