The cooling water filling the reactor vessel, heated by the core, gets hotter and its pressure rises. A relief valve at the top of the pressurizer tank - the so-called pilot-operated relief valve (PORV) - opens automatically, steam escapes. But the temperature and pressure of the water in the primary system continue to rise because heat generated by uranium fission in the reactor's core is no longer being removed from the reactor cooling water in the steam generator.

[La centrale Three Mile Island-2 et sa conception] Le pressuriseur du circuit primaire était placé sur la branche chaude de la boucle A; une vanne de décharge électropneumatique (PORV : Pilot-Operated Relief Valve) était installée dans la partie supérieure du pressuriseur. Cette conception des GV [générateurs de vapeur] entraînait un inventaire en eau du circuit primaire notablement inférieur à celui des réacteurs ayant des échangeurs de chaleur à tubes en U (cas des réacteurs REP [réacteurs à eau pressurisée] français), et rendait donc le réacteur un peu plus sensible aux transitoires.
