En 1970, Robert Machemer, avec l'aide d'un biophysicien, Jean-Marie Parel, développa la vitrectomie à une seule voie : le vitréotome assurait à la fois la coupe, l'aspiration et l'infusion. En 1972, une lumière froide fut associée au vitréotome. La même année, O'Malley sépara les différentes fonctions en introduisant l'infusion isolée, avec une endo-illumination par une autre voie apportant un éclairage de bonne qualité.
