L'élastofibrome n'est pas une véritable tumeur mais une pseudo-tumeur dégénérative du tissu élastique avec production excessive de collagène et de fibres élastiques anormales, en réponse à des microtraumatismes répétés dus à la sollicitation excessive des articulations de l'épaule. Il s'agit plus d'une élastodysplasie que d'une dégénérescence du tissu collagène. L'élastogénèse est imparfaite, avec blocage dans le processus de maturation normale.
