Chez les clients atteints de la forme précoce de la MA [maladie d'Alzheimer], trois gènes jouant un rôle important dans l'étiologie de la maladie ont été identifiés : l'apolipoprotéine E4, la préséniline 1 (PS1) et la préséniline 2 (PS2). Le premier gène que les chercheurs ont associé à la MA est le gène de l'apolipoprotéine E4 (gène apo E4) sur le chromosome 19. L'apo E se présente sous différentes formes (allèles), dont trois sont plus courantes; les gens héritant d'un allèle de chacun de leurs parents (p. ex. l'apo E2, l'apo E3, l'apo E4). L'apo E sert au métabolisme des lipoprotéines et semble agir sur l'élimination des plaques amyloïdes. Les mutations sur ce gène entraînent une augmentation des dépôts d'amyloïde. Ainsi, la présence de l'apo E4 est associée à une augmentation du risque de souffrir de la forme tardive de la maladie. Toutefois, la présence seule du gène ne suffit pas à expliquer la maladie, car nombre de personnes porteuses du gène apo E4 ne souffriront pas de la MA.
