Proximal convoluted tubule diuretics. The proximal convoluted tubule is the primary site for NA+ [sodium ion] reabsorption and seems a likely site for effective diuretic action. However, the number of downstream reabsorption sites limits the effectiveness of any diuretic targeting this region. Carbonic anhydrase inhibitors are the only commonly used diuretics that particularly target this region although they are rarely used primarily as diuretics. They work by blocking the enzyme carbonic anhydrase, which acts to help reabsorb bicarbonate in the proximal tubule.

Acetazolamide is an example of a proximal diuretic.

Inhibition of enzyme action based solely on the concentration of the inhibitor; substrate concentration is not a factor. The inhibitors combine with the enzyme on a site other than that utilized by the substrate.

In non-competitive inhibition the binding of the inhibitor substance is thought to induce a conformational change in the enzyme molecule which does not interfere with the attachment of the substrate to the active site, but which greatly reduces the catalytic activity of the enzyme.

Le type le plus courant d’inhibition non compétitive est produit par les réactifs qui peuvent se fixer réversiblement, hors du site actif, sur un groupement fonctionnel de l’enzyme indispensable au maintien de la conformation tridimensionnelle active de la molécule enzymatique. Certains enzymes possédant un groupement -SH essentiel sont inhibés de façon non compétitive par les métaux lourds. Ces groupements -SH doivent donc être intacts pour que l’enzyme conserve sa conformation active normale.

L'acétazolamide, diurétique fort utilisé, agit en inhibant de façon non compétitive l'anhydrase carbonique au niveau du rein.
