The inferior oblique muscle has its origin on the maxillary bone just posterior to the inferior medial orbital rim and lateral to the nasolacrimal canal. The inferior oblique is the only extraocular muscle to have its anatomic origin in the anterior orbit. The muscle runs from the medial corner of the orbit to the lateral aspect of the globe, its length approximately paralleling the tendon of insertion of the superior oblique muscle.

The superior oblique is one of the two noteworthy oblique extraocular muscles. These muscles are unique in that they do not originate from the common tendinous ring, have an angular attachment to the eyeball, and they attach to the posterior aspect of the eyeball. The superior oblique functions explicitly to move the eye in the down-and-out position and intort the eye.

The adult eyeball measures about 2.5 cm (1 in.) in diameter. Of its total surface area, only the anterior one sixth is exposed; the remainder is recessed and protected by the orbit, into which it fits. Anatomically, the wall of the eyeball consists of three layers: fibrous tunic, vascular tunic, and retina.
