Il existe deux isoformes de chondroïtine sulfate : la forme 4-sulfate et la forme 6-sulfate. Dans les cartilages de sujets jeunes, la forme 4-sulfate prédomine sur la forme 6-sulfate, alors que c'est l'inverse dans les cartilages de sujets âgés. Plus le taux de chondroïtine-4-sulfate est bas et celui de 6-sulfate est élevé au sein du cartilage articulaire, plus le métabolisme des protéoglycanes décline. Le ratio chondroïtine-6-sulfate sur chondroïtine-4-sulfate serait donc le reflet du degré de gravité de l'arthrose.
