L'aspect papillaire est commun à l'épendymome et au plexus choroïde. C'est un assemblage d’axes conjonctivovasculaires ramifiés, revêtus de cellules cubocylindriques claires, sans cil apical. De tels axes peuvent contenir des tubes et des cordons à l'épithélium de même structure, ainsi que des calcosphérités. Quand ils subissent une dégénérescence mucoïde, on parle d’épendymome myxopapillaire; ce qui se rencontre, avant tout, au niveau du filum terminal spécialement chez le sujet de 30 à 50 ans.
