We report two histopathologically proven cases of epithelial downgrowth in which the secondary glaucoma was controlled by a double-plate Molteno tube shunt and useful vision restored with a penetrating keratoplasty. L'invasion épithéliale de la chambre antérieure peut se faire le long d’une suture, volontiers à la soie, perforante, trop profonde ou trop serrée, le long d’un tissu étranger incarcéré dans la plaie(iris, vitré, capsule) ou enfin au travers d’une plaie fermée par invagination d’une lèvre cruentée de l'épithélium cornéen ou d’un lambeau conjonctival. Cliniquement, l'œil reste généralement calme les premières semaines et ce n’ est que progressivement que l'on voit apparaître un feuillet blanchâtre translucide, parfois même transparent, qui tapisse l'iris, la face interne de la cornée et s’organise en formant un ou plusieurs kystes dans la chambre antérieure. Cette membrane est très souvent reliée à l'orifice d’entrée(incision, suture, fistule). Ceci aboutit progressivement à la perte fonctionnelle du globe et il est généralement difficile de réparer cette anomalie de cicatrisation.
