The modernized confocal microscope is more sensitive than the commercial confocal microscope. The laser intensity can be reduced, which decreases photobleaching and damage of the specimen and consequently allows experimenting with living cells. La focalisation très précise réduit la profondeur de champ et permet d’obtenir une image dans un plan focal d’épaisseur très mince. En faisant varier la distance focale, il est alors possible de «découper» l'objet examiné en une suite de plans à différentes profondeurs, dans les limites de transparence du matériau. On utilise par exemple la microscopie confocale pour étudier la peau humaine in vivo.
