Par remplacement de l'acide acétique de la molécule d’acétylcholine par l'acide carbamique on obtient le carbachol ou carbaminoylcholine qui est relativement résistant aux cholinestérases et devient ainsi plus actif mais aussi plus toxique. Il possède des actions muscariniques et nicotiniques. Il a été utilisé par voie locale, sous forme de collyre, dans le traitement du glaucome mais, en raison de sa toxicité, il ne l’est plus.
