A biopsy of normal sized kidneys showed diffuse glomerular mesangial sclerosis with cellular proliferation, exsudative lesions, large capillary plugs with hyaline material. Deposits were negative with the Congo red stain. There was a diffuse interstitial fibrosis with some patchy lymphocytic infiltrate and frequent arteriolar hyalinosis. No myeloma casts were seen.

La glomérulosclérose diffuse intercapillaire. Elle précède dans le temps les lésions nodulaires auxquelles elle est constamment associée. Elle réalise une infiltration du tissu mésangial par des dépôts éosinophiles PAS positifs [coloration acide périodique de Schiff] qui entraînent une réduction progressive du calibre des capillaires glomérulaires.

OBS

Le mésangium, ou espace intercapillaire, est formé de cellules mésangiales et d'une substance intercellulaire (de nature proche de la membrane basale glomérulaire).
