La goniotomía (operación de Barkan) es la técnica de elección en el glaucoma congénito. En esta operación se realiza una incisión arcuada, utilizando un bisturí especial, a través de la trama trabecular, a un nivel intermedio entre la raíz del iris y la línea de Schwalbe, de modo que se secciona la membrana de Barkan (tejidomesodérmico persistente en el ángulo camerular que impide el drenaje hacia el canal de Schlemm).
