L'exagération du réflexe achilléen s'accompagne habituellement d'un clonus du pied qui se recherche de la façon suivante : on saisit l'avant-pied et on fléchit brusquement le pied sur la jambe; on provoque une succession rythmée de mouvements d'extension du pied, persistant aussi longtemps qu'on maintient une certaine pression sur l'avant-pied. Ce clonus inépuisable ou clonus vrai diffère du faux clonus, fait de quelques secousses vite épuisées.
