A surrogate endpoint is a laboratory measure or a physical sign that is intended to be used as a substitute for an endpoint measuring a direct, meaningful clinical outcome (clinical outcome assessment or COA). A surrogate endpoint does not measure the expected clinical benefit of the therapy being studied directly, but rather is expected to predict that clinical benefit. For example, intraocular pressure is a surrogate endpoint for loss of vision in studies involving therapies to treat glaucoma.

Refers to a judgment made by a surrogate decision-maker on behalf of an incompetent individual according to what the surrogate decision-maker believes would be the wishes of the incompetent individual were the individual competent.

The primary purpose of the substituted judgment test is to attempt to determine what decision the mental incompetent would make, if she were reviewing her situation as a competent person, but taking account of her mental incapacity as one factor in her decision.
