Le fosinopril est un inhibiteur de l'enzyme de conversion (IEC) de l'angiotensine I en angiotensine II, substance vasoconstrictrice mais également stimulant la sécrétion d'aldostérone par le cortex surrénalien. Il en résulte : une diminution de la sécrétion d'aldostérone; une élévation de l'activité rénine plasmatique, l'aldostérone n'exerçant plus de rétrocontrôle négatif; une baisse des résistances périphériques totales avec une action préférentielle sur les territoires musculaire et rénal, sans que cette baisse ne s'accompagne de rétention hydrosodée ni de tachycardie réflexe, en traitement chronique. L'action antihypertensive du fosinopril se manifeste aussi chez les sujets ayant des concentrations de rénine basses ou normales. 3, record 1, French, - fosinopril
